What kernel are you using? Are you also using the FTP helper module? There was a sudden surge in discussion of that breaking/invalidating persistence templates during the last 18 months or so - search the list archives if you want to see the threads.

In a nutshell, if you used the FTP helper module (in a NAT platform) and had persistence switched on, more often than not the persistence would be flaky at best. It got fixed, ooh, maybe a year ago.
