I believe that this can be achieved by running fault tolerant real-servers,
that is two real-servers running in lock-step. Two solutions that
I am aware of that allow that to be done are Kemari and Remus, both of
which. I believe that both of those solutions work in conjunction with Xen
and that a version of Kemari for KVM is also available.
